Once you have answered and submitted the above New Client Questionnaire, please click on the red button to the right.  This button will open a new screen, which contains the Informed Consent Form.   This form should be read and then signed, all of which can be easily done online in minutes.  Afterwards, we will contact you (though you're welcome to contact us) to speak with one another and schedule an appointment time.
